If someone took someone's mod and heavily edited it in Blender and then uploaded it to the official modhub is that considered stealing?

The only time that you can use other peoples mods whether part or whole, is if you ask for permission and he/she gives you permission, and even then they would want to see it first before they give you permission to use their mod, and you must give them the credit in the writeups for his/her part in the creation of the mod.
If they say no, you are only allowed to use it in your own private game and nowhere else.

now if someone was to have taken assets from another game should they be reported?

You should get the permission of the othe games developer, this is somewhat common in other games like fallout for example.
